1. Firstly there is no requirement for the players to go in the same order the second time around. So realistically it doesn't become an issue until all his team mates have taken one more kick him than him. Assuming that is the case, he can't be substituted. So I'd say he has to take.
2. Book him and send him. Clearly delaying the restart of play. Stupid play.
3. Any hand shakes are protocol as opposed to law. Not much you can do about it. File a report.
EDIT: On second thought, if it's an obvious and visible action it could be considered dissent by action so you could caution. I wouldn't though.
