I think some people put players who are suspended in there, figuring that they are suspended for the league, not this competition - but leave injured players out. It is a bit of an inconsistent unwritten rule...I take out all injured and suspended players, but that is also partially because I'm afraid I'll forget to do it before the league game on Wednesday :p
Also some people do just genuinely forget, especially if they had a cup game on a Monday night.

3-0 up after 15 minutes somehow against Sanchez, it ends 3-2, including a 92nd minute consolation. I've gone three points clear at the top, I'm a little concerned that I'm going to get promoted here.
